REFERRAL PROCEDURE FOR CREATIVE PATHWAYS

We anticipate that most of our referrals will take the form of Self-referral.

Contact us on the free-phone number supplied. A member of our administration team will take this call, and you will be asked a series of questions related to your personal details, diagnosis and treatment. Please be reassured that all information will be dealt with in the strictest confidence, and no third party will be privy to your information.

Please also be aware that, as you will be dealt with by an administrator, with no clinical expertise, any threats of harm to yourself or others will either be directed to the appropriate service (e.g. Police, Ambulance) or you would be advised that your call cannot be dealt with at the present time.

Third Party referrers (e.g. CPN, Social Worker or GP) can also contact us via the free-phone number, after receiving permission from you, the client, to pass on your details. Third party referrers can also fax us with completed referral forms, which we will supply on request.

Once we have received your referral, and have confirmation of your payment details, we will then contact you within 24 hours and discuss the care package you require.
